Tympanoplasty Surgery Cost Guide in India

Tympanoplasty is a surgical procedure to fix a perforated or opened tympanic membrane (eardrum), or the small bones in the middle of the ear (ossicles in the mastoid bone). Repeated ear infections, trauma, or frequent ear surgery, can cause damage to the eardrum and may result in hearing loss. Depending on the requirement, the tympanoplasty may involve ossiculoplasty (reconstructing the bones located at the back of the tympanic membrane).

Tympanoplasty – Eligibility 

Following are the eligibility criteria for tympanoplasty: 

  • Patients in the age group of 10-60 years.
  • Patients with chronic ear problems that cannot be treated with medications.
  • Patients with conductive hearing loss.

Individuals with immune disorders or chronic smokers are ineligible for tympanoplasty. 

Tympanoplasty – The procedure

Initially, a physical examination is conducted, and medical history is reviewed to assess the patient's health. An audiogram and other hearing tests like pneumatic otoscopy and otomicroscopy are determined the problem in the ear. Based on the test results, the procedure and grafting techniques are finalized. 

Tympanoplasty is of five types. They are:

  • Type I (myringoplasty):  The perforated area in the eardrum is restored by a graft.
  • Type II:  The tympanic membrane perforations, along with the erosion of the malleus (hammer-shaped bone in the middle ear) are restored by a graft. Grafting is done on top of the incus (a bone located in the middle ear) or remnants of the malleus.
  • Type III:  Grafting is done on top of the stapes (a bone in the middle ear) to provide protection. This method is used when there is a destruction of two ossicles.
  • Type IV: The graft is placed around or onto the mobile stapes footplate (present on the right side and head of the stapes). This method is used in case of ossicular destruction (including all or a part of stapes arch).
  • Type V:  The procedure is useful when the footplate of the stapes bone is fixed. 

Depending upon the type, either local or general anesthesia is administered. If the perforation is small, a small incision is made into the ear canal, and the remaining eardrum will be uplifted away from the canal. A microscope is used to magnify the view of the internal structure to help operate.  

If the perforation is large, then the incision is made behind the ear, which brings the entire ear forward and gives the surgeon full access to the perforation. If any scar tissue is found, it is removed using a laser or micro hooks.  For grafting, tissues are taken from the tragus (a small cartilaginous lobe of skin in front of the ear) or from a vein.  Once the grafts are placed in position, an absorbable gelatin sponge is placed under the eardrum to support the graft. Finally, the incision is closed with stitches, and a sterile patch is positioned outside the ear canal.

You can go home after two to three hours of surgery. Follow-up is required a week after the surgery to examine the graft and to remove the packing.  You may need to stay for at least seven to 10 days for undergoing tympanoplasty in India.
