Kidney Transplant in India

Chronic kidney disease is one of the major health concerns affecting 10% of the population worldwide. It is a "progressive" disease and becomes worse over time and may lead to kidney failure. When kidney function declines to a certain level, patients have end-stage renal disease and require either dialysis or transplantation to sustain their life. 

Kidney transplant or renal transplant is a surgical procedure in which a healthy kidney from a living or deceased donor is transplanted into the patient with end-stage kidney disease. Kidney transplant surgery is considered safe, and it increases the chances of living a longer and better life.

Who is a good candidate for a kidney transplant?

Any adult or child who has been diagnosed with stage 5 chronic kidney disease or end-stage renal disease can be considered for transplant evaluation. However, there are some requirements and criteria that a person must meet to qualify:

  • Should be healthy enough to withstand a serious surgery and a lifelong medication routine.
  • Should be a non-smoker, and avoid alcohol and drugs 
  • Should be willing and able to take medications as directed by the transplantation team

In some cases, kidney transplant cannot be performed if the patients have:

  • Conditions like cardiovascular disease liver diseases, cancer, infections like hepatitis, tuberculosis, or bone infection
  • History of serious psychiatric disorders that cannot be successfully treated before transplantation
  • A habit of consuming alcohol or abusing drugs 
  • Have any medical conditions that would interfere with transplant success

What will happen at the initial consultation?

During your initial consultation, you must have a complete health evaluation by the transplant team to decide whether you are eligible for the kidney transplant. The surgeon will review the medical history and a complete medical examination is done to evaluate the suitable procedure. The evaluation tests include:

  • Blood and tissue type tests
  • HIV and hepatitis tests
  • Prostate examination for men
  • Mammogram and pap smear for woman
  • Colonoscopy
  • Heart and lung exams
  • Kidney and liver test

The kidney transplant evaluation also requires a mental health evaluation to make sure that you are mentally ready for the transplant and can follow the entire post-procedure directions.

If you are eligible for the transplant, you may be added to the national waiting list for a donor kidney. If you have a living kidney donor (a person who wants to donate their kidney to you), you may have your transplant as soon as both you and your donor are ready.

How is kidney transplant performed?

The first step for kidney transplantation will be finding a suitable donor for the patient.  The following factors are considered while selecting a suitable kidney donor:

  • Blood type testing:  The first test establishes the blood type. The blood type of the donor should be compatible with the patient's blood type for the transplantation.
  • Tissue typing: The donor will be tested for HLA (Human Leukocyte Antigen) typing. This test matches genetic markers that indicate the possibility that the transplanted kidney will function efficiently in the patient's body.
  • Crossmatch: This matching test involves mixing a small sample of patient blood with the donor's blood in the lab. The test determines whether antibodies in your blood will react against specific antigens in the donor's blood. A negative crossmatch result indicates compatibility.

Transplant surgery

Once you get to the hospital, you will be shifted to the procedure room. Before the transplant, you will be given anesthesia (general anesthesia) to make you sleep during the procedure.

During the procedure, the surgeon will make an incision on your abdomen. The new kidney will be put into your body in this area, and the kidney's blood vessels are attached to the blood vessels in the lower portion of the abdomen, right above one of the legs. The ureter (the tube that carries urine from the kidney) from the new kidney is attached to the patient's bladder. The surgeon will close the skin incision and you will be taken to the recovery room. The procedure typically takes two to four hours to perform.

After the transplant surgery, you will have to stay in the hospital where you will be closely observed for signs that your body is accepting the new kidney. A stay of 40 days is required for undergoing kidney transplant surgery in India.
